Developmental Center Staff Training
In addition to family members, developmental center staff members are an integral part of the transition process. Many of these staff members have been supporting your family member and know them very well. CLEP is involved in training developmental center staff members on the transition process, ELP model, their role as part of the core team, and all the possibilities and options available in community living today.
Provider Agency Training
Provider agency staff members are trained on the community transition process and how to successfully support individuals who are transitioning to the community. Staff members are trained step by step on how to utilize the ELP and how support needs are revealed in this plan, while allowing individuals to experience self-determination through their own choices.
Person-Centered Thinking (PCT) and Essential Lifestyle Planning (ELP)
CLEP provides PCT/ELP training for individuals, families, support coordination agencies, case managers, and provider agencies. PCT training serves as a foundation for all who are involved in supporting people with disabilities in New Jersey. All trainers who deliver PCT/ELP have been credentialed by The Learning Community for Person Centered Practices.
DDD uses a person-centered planning tool called the ELP. This person-centered plan built around your family member is designed to identify the services and supports he/she will need to create a life of choice and thrive in community living. The plan balances what is important TO your family member based on his/her likes and dislikes and what is important FOR your family member based on his/her needs and abilities, in order to be healthy and safe.
